Clinical and radiological findings: Terrible Triad: Surgery after 5 days, coronoid tip fracture and torn MCL left alone, radial head replacement + LCuL suture anchors reinsertion. Intraop fluoro and Postop Plain film XR showed joint congruence. X rays at 20 days demonstrates Humeroulnar subluxation upon active extension around 60deg.
Planning remarks: Pre-op Fluoro: medial opening + drop sign. Therefore, we've decided to go medial to address MCL without revising the LCL repair.
Patient positioning: Supine
Anatomical surgical approach: Medial standard approach into FCU split
Operative remarks:MCL augmentation with Internal Brace.
Intraop Fluoro: No drop sign along the entire arc of movement, medial stability
Orthopaedic implants used: Internal Brace (Arthrex)

Trauma 20 days ago. Surgery 15 days ago, coronoid tip fracture and torn MCL leave It alone.
There Is enough room for a LCL reconstruction with palmaris longus (i.e. docking technique) and an IJS for protection?@Gnomelover1970 @traumaticum #orthotwitter pic.twitter.com/hV9YBfiMis
