Clinical Details

Clinical and radiological findings:  Terrible Triad: Surgery after 5 days, coronoid tip fracture and torn MCL left alone, radial head replacement + LCuL suture anchors reinsertion. Intraop fluoro and Postop Plain film XR showed joint congruence. X rays at 20 days demonstrates Humeroulnar subluxation upon active extension around 60deg.

Preoperative Plan

Planning remarks:  Pre-op Fluoro: medial opening + drop sign. Therefore, we've decided to go medial to address MCL without revising the LCL repair.

Surgical Discussion

Patient positioning:  Supine

Anatomical surgical approach:  Medial standard approach into FCU split

Operative remarks: 

MCL augmentation with Internal Brace.

Intraop Fluoro: No drop sign along the entire arc of movement, medial stability

Orthopaedic implants used:   Internal Brace (Arthrex)
