Clinical and radiological findings:  Trimalleolar tibial pilon open fracture - Soft tissue coverage defect - chronic wound - Osteomyelitis

Preoperative Plan

Planning remarks:  Staged management

Surgical Discussion

Operative remarks: 

Radical debridement.

Resection osteotomy of exposed bone tissue.

Acute distal shortening.

ILIZAROV External Fixation- Tibio Talar Arthrodesis - Diaphyseal osteotomy for progressive bone lengthening.

Training on the go.

Bone lengthening of 4 cm - 1 mm per day.

Early conversion to tibial nail Retrograde PROTEC of antibiotic release from J&J.

ARTHRODESIS calcaneus cut tibial plus protection of regenerated bone after cleaning the medullary canal with the RÍA system from J&J.

Cure of PSEUDOARTHROSIS in the docking site area - autologous bone graft from the iliac crest plus BMP morphogenetic protein - Medtronic.

ANTIROTATORY augmentation plate.
