Clinical and radiological findings:  Split-Depression Posterolateral

Preoperative Plan

Planning remarks: 

Surgical Discussion

Patient positioning:  Supine

Anatomical surgical approach:  Posterolateral - Posteromedial

Operative remarks: 

Through the posteromedial approach, pes anserinus tendons were detached from their insertion, and posterior buttress plating was performed in the 'figure 4' position of the knee .

Through the posterolateral incision, depressed fragment was elevated and void filled with allograft.

Finally, lateral to medial and anterolateral-to-posteromedial rafting screws were applied.

Excellent result. Shows you can go a long way laterally from a Lobenhoffer. Personally not had to release Pes before though. Huge depression. How did it hold up at follow-up?

Was it an error, may be popliteus and not pes

This patient was operated on three weeks ago. Everything seems to be going well for now. There are no signsdepression or any other complications at the moment. And yes It is PES. Initially, I attempted to preserve it. However, due to its narrowing the area I released it. .
