Clinical Details

Clinical and radiological findings:  91-year-old diabetic woman with a 20-year-old total knee replacement that was working well. She suffers a fall with pain and impotence of her knee

Surgical Discussion

Operative remarks: 

The surgical planning was an intramedullary nail with a locked lateral plate vs. double plate. A CT was performed where comminution and poor quality bone were observed, in addition to the fact that it was a PFC -PS (De Puy) TKA without the possibility of retrograde intramedullary access, so the second option was decided. During surgery, the distal fragment was reduced with provisional KW by simultaneous double approach to aid reduction. the medial and lateral plates were fixed alternately

Excellent job. Great to see soeone else using the Synthes MDTP on the medial side. best plate i found so far for that location. I find if you exaggerate the bend a little it can sit a little further down on the condyle, plus the screws shoot a little more distally and not so horizontally.
